Friends & Family Test

We would appreciate if you would complete our friends and family survey. You can find the leaflets in your GP Surgery. Updating contact details

It is important that your contact details are kept up to date, when booking an appointment you will be asked to confirm your contact information. If you have changed address please visit your Practice and ask for a change of address form, or alternatively fill in the form at the bottom of this screen.

Out of Hours

If the GP surgery is closed and you require medical attention please contact NHS 111 who will be able to give advice and locate the best service for you.

Nurse Practitioners

Ms Tracy Pettitt (f)

Tracy Pettitt is the Clinical Lead Nurse for ELMS Federated Practice, Tracy works Monday through to Thursday and has clinics across all four of the sites.

Tracy qualified as an Advanced Nurse Practitioner in 2005 and followed on to gain further qualifications in women's sexual health, family planning, diabetes and prescribing. Tracy's specialist interests include paediatrics, dermatology and gynaecological problems.

During Tracy's working week she will help to cover triage for home visits, attend acute home visits and assist the GP with the emergency oncall clinics.

Based at Pendle Valley Mill Practice, Monday, Tuesday, Thursday and Friday.

Based at Brierfield, Monday to Friday as the Over 75s Community Matron.

Based at Pendle Valley Mill Practice on Tuesday and Wednesday, based at Brierfield Surgery on Thursday and Friday.

Specialist Nurses

Mrs Sue Hall (f)

Sue Hall qualified as an RGN Nurse in 1994 and commenced her role as an Out of Hours Triage Nurse, until 2001 when Sue started work at NHS Direct as a Senior Nurse Advisor, Sue remained at NHS Direct until 2012 when she joined the team at ELMS to become the Triage Nurse for the Federated Practice.

In her role as Triage Nurse Sue discusses and organises the home visits to be placed with the appropriate clinician, provides advise when needed and helps to maintain regular reviews for palliative care patients.

Sue is based at St Ive's House with the HUB (booking line) whilst completing her Triage list she also helps the HUB staff with any queries that may get asked from the patients and points the patients to the appropriate clinician.

Qualified in 1994 as registered General Nurse and qualified with Professional Diploma of Professional Studies (Health Studies) in 2003. I have Diploma level qualifications in COPD, Spirometry, Chronic Heart Disease, Diabetes and and Insulin Conversion of Type 2 Diabetics Also trained in cervical smears, travel health, baby immunisations and NHS health checks. I am a student nurse mentor/assessor and a non-medical prescriber and am due to graduate in July 16 with BSC (Hons) Prefessional Practice. I have 13 years experience as a practice nurse.

Practice nurses are qualified and registered nurses. The practice nurses run clinics for long-term health conditions such as asthma or diabetes, minor ailment clinics and carry out cervical smears.

Healthcare assistants support practice nurses with their daily work and carry out tasks such as phlebotomy (drawing blood), blood pressure measurement, ECGs and new patient checks. They may act as a chaperone when a patient or doctor requests one.
