Friends & Family Test

We would appreciate if you would complete our friends and family survey. You can find the leaflets in your GP Surgery. If you would like to leave a comment about the service provided by your Practice you can add your comments onto the NHS choices website. Follow the link and search for your Practice 

Updating contact details

It is important that your contact details are kept up to date, when booking an appointment you will be asked to confirm your contact information. If you have changed address please visit your Practice and ask for a change of address form, or alternatively fill in the form at the bottom of this screen.

Out of Hours

If the GP surgery is closed and you require medical attention please contact NHS 111 who will be able to give advice and locate the best service for you.

Dear Patient


With patients’ needs at the heart of everything that we do, ELMS Federated Practice would like to know how we can improve our service to you and how you perceive your surgery and staff.  


We recently invited all patients to join a Patient Participation Group, to represent patient’s interests, to help your Practice be truly patient focused and to improve service delivery at each of the branch surgeries.  The Patient Participation Group has now been established and meets on a monthly basis to discuss Practice Matters with staff representatives from the Practice.  To see an example of the types of things which are discussed, please see copies of the minutes of the last meeting below:

If you would like your Practice Patient Representatives to raise any service improvement, If you have suggestions or you would like to pass on compliments, please ask reception staff for an envelope and leave a note for us in the Patient Suggestions Box.


There are still opportunities to join as a member of the PPG and to attend meetings. If this is something you would like to consider, please email: for more information.


We are also setting up a ‘virtual’ patient representation group so that all patients can have their say. We will ask the members of this representative group to contribute by email or telephone. For example we may contact you from time to time, to ask for your opinions on practice related matters, which may include: “what you think about our opening times or the quality of the care or service you have received from us.” To achieve this we may occasionally contact you via email or telephone, always mindful of your time and of course, your confidentiality.


Our aim is to gather as many patient views as possible, from as broad a spectrum as possible, to get a truly representative sample of all the Patients in our diverse Community. We need Young people, The Employed and Unemployed, Retirees, People with long term Health conditions, people from non-British ethnic groups and people from the Travelling communities to contribute so that we can gather their views.
