Flu Vaccinations

If you are eligible for a flu vaccination, please contact the surgery or booking line to book your appointment.

Friends & Family Test

We would appreciate if you would complete our friends and family survey. You can find the leaflets in your GP Surgery. If you would like to leave a comment about the service provided by your Practice you can add your comments onto the NHS choices website. Updating contact details

It is important that your contact details are kept up to date, when booking an appointment you will be asked to confirm your contact information. If you have changed address please visit your Practice and ask for a change of address form, or alternatively fill in the form at the bottom of this screen.

Out of Hours

If the GP surgery is closed and you require medical attention please contact NHS 111 who will be able to give advice and locate the best service for you.

Repeat Prescriptionsmedication7

 

If you are taking regular medication we will give you a computer slip listing your medication. When you next require a prescription either hand in your slip at reception marking the items you need, send it with a stamped addressed envelope for return by post or you can fax it.

Alternatively you can arrange an agreement with your local pharmacist collection service.

Please note we do not take prescription requests over the phone. Furthermore we will only give a repeat prescription to a representative of a patient where we are satisfied that the patient has given his or her consent for the representative to collect the prescription.

Medication Reviews

The doctor may request that you attend for an appointment before prescribing medication for the first time or where a48_hours course of medication has expired.

Whenever possible please request your repeat prescription before 11:00 for collection 48 hours later.
