Flu Vaccinations

If you are eligible for a flu vaccination, please contact the surgery or booking line to book your appointment.

Friends & Family Test

We would appreciate if you would complete our friends and family survey. You can find the leaflets in your GP Surgery. If you would like to leave a comment about the service provided by your Practice you can add your comments onto the NHS choices website. Updating contact details

It is important that your contact details are kept up to date, when booking an appointment you will be asked to confirm your contact information. If you have changed address please visit your Practice and ask for a change of address form, or alternatively fill in the form at the bottom of this screen.

Out of Hours

If the GP surgery is closed and you require medical attention please contact NHS 111 who will be able to give advice and locate the best service for you.

Appointments

circled_dayIn order to reduce ‘did not attend appointments’ (DNAs)  the practice has adopted a weekly appointment system where you can book one week in advance.

Please note a separate appointment should be booked for each person and each problem. Double appointments may be booked for complicated problems.

If you are over ten minutes late for your appointment you will not be seen and will have to make another appointment.

Chaperones 

If you feel you would prefer a chaperone (relative or friend) while being examined, please inform the receptionist in advance.

Telephone Availability

If you want to speak to the doctor, please ring to arrange a telephone consultation. If the doctor or other clinicians are unable to take the call, your details will be taken down by the receptionist and passed on to the doctor.  Every effort will be made to ring you back the same day. 

Home Visits 

If for medical reasons you are unable to attend the surgery, please try and ring before 10:30. If you require a home visit.  This includes rest and nursing homes.

Cancellations

If you cannot attend your appointment for any reason please inform us in good time so that the slot can be given to someone else. If you fail to attend an appointment without notice a letter will be sent to you and a copy will be stored in your records. If you fail to attend consecutive appointments, you may be removed from our list and asked to register with another practice.
